Where can I find a free Medical Power of Attorney forms specific to the State of Connecticut?

People were helpful before in finding me a GENERAL Power of Attorney form for State of CT at

….but I can’t seem to find a MEDICAL Power of Attorney form at the same website.

Does anyone have other suggestions as to where to find a CT Medical Power of Attorney form to download?
